This course will focus on the experiences of complex patients and pathways within the NHS Trust and how they work with key external partners, and the importance of undertaking safeguarding impact assessments to identify what systems and processes are in place to address any variations and health inequalities that have safeguarding implications and how this fit in within the context of the NHS Long Term Plan. It focuses on how complex patients who are medically optimised but remain within acute settings due to a lack of suitable placement are supported using a system wide risk management approach, and how a safe discharge can be facilitated to an more appropriate care setting and what commissioning arrangements and pathways should be in place.

1. Learn about the lack of pathways and adequate support for people with complex needs who have dementia, mental health and LD and or autism and how this remains recurrent themes as indicated locally and nationally in published safeguarding adults reviews, section 42s and serious incidents.
2. Understand the implications of this for NHS acute settings, where patients may remain for a significant length of time despite being when medically optimised and or are admitted frequently due to a lack of adequate services and support within the community, which would be required to prevent readmission.
3. Key challenges faced by NHS organisations in relation to supporting patients with complex needs within an acute setting and the impact this has on the care of the person.
4. Highlights the gaps in commissioning pathways, as well as the role of commissioners in supporting providers in addressing these issues and what is required within pathways to address these challenges.
5. Draws from the key recommendations as outlined in CQC reports relating to how people with complex needs, such as enduring mental health needs are met within acute hospitals, and how can this be improved.
6. Focus on gaps in governance and systems and processes relating to this and what NHS organisations are required to have to mitigate these gaps to ensure that the care of people with complex needs is effective and safe with clear leadership in place.
