This include those that relate to patient safety, care of people with mental health, learning disabilities and or autism within acute general hospitals and the pathways required within the community to minimise recurrent hospital admissions. Dementia Care – we support organisations with all aspects relating to dementia care issues to ensure that the organisation has the systems and processes in place to support people with dementia as outlined in best practice guidance.
